Telehealth Consent

Telehealth services allow you to receive healthcare through secure electronic communication technologies when you and your healthcare provider are in different locations.

Telehealth may offer several benefits, including:

- Improved access to healthcare services

- Increased convenience and flexibility

- Reduced travel time and related expenses

- Timely access to medical evaluation and treatment

However, telehealth also has limitations. Certain medical conditions may require an in-person examination, diagnostic testing, or emergency treatment that cannot be provided through telehealth. Additionally, technical issues such as internet outages, equipment failures, or communication disruptions may occasionally affect the quality or availability of services.

By scheduling or participating in a telehealth appointment with Priority Care Telehealth, you acknowledge that:

- You understand the benefits and limitations of telehealth services.

- You consent to receiving healthcare services through telehealth technology.

- You are responsible for providing accurate and complete health information.

- You confirm that you are physically located in a state where Robin Bohanon, FNP-C, is licensed to provide care at the time of your appointment.

You may withdraw your consent to telehealth services at any time. Please note that withdrawing consent may limit or prevent the provider's ability to deliver healthcare services through telehealth.

If you have questions regarding these Terms of Use, Privacy Policy, or Telehealth Consent, please contact Priority Care Telehealth before using our services.
